You are placing an order for a new Team Costume. Please fill out this form carefully and we will be in touch to send you the receipt of the payment for your records. A deposit of £100 for Team Costume and £50 for Waistcoat is required to secure your order. The card you submit here will also be used to charge the balance of the costume when it is due.
